I certify that, to the best of my knowledge and belief, I am in good health and in proper physical condition for the anticipated conditions of the event, and I agree that it is my sole responsibility to determine whether I am sufficiently fit and healthy enough to participate. I understand and acknowledge the physical and mental rigors associated with swimming and realize that such events are inherently dangerous and represent an extreme test of a person’s physical and mental limits. I voluntarily and knowingly recognize, accept and assume these risks. I also accept sole responsibility for my own conduct (and/or on behalf of my child) and actions while participating in this event.
To the fullest extent permissible under law, I, individually (and/or on behalf of my child), agree to waive and to release Badger Swim Club from any and all claims, suits, causes of action, judgments or settlements for injuries, up to and including death, loss of property and any and all other damages of any nature whatsoever arising out of my (or my child’s) attendance at Badger Swim Club, use of its facilities or engaging in other activities at Badger Swim Club. Having read this agreement in full I enter into it with full knowledge of its consequences.

During Badger Swim Club’s Stroke Clinics, we may take photographs of activities involving swimmers. Some photographs may capture your child’s participation, directly or indirectly. These photos may be published through our website, social media pages, and news bulletins. With this, we seek for your consent in allowing us to publish photos which may involve your child to the said platforms.
